Long-Term Care Benefit in Cash for a Foreign Caregiver
Israel's long-term care benefit is paid by default as services — care hours delivered through a care company. Families employing a foreign caregiver can instead request most of the benefit in cash, paid into the patient's bank account and used toward the caregiver's wage.

Services vs. cash
- Benefit as services (default): National Insurance pays a care company, which supplies care hours at home.
- Benefit as cash: the amount goes to the patient's bank account and the family manages the care, including paying the caregiver.
- Employing a foreign caregiver is the clearest case for the cash track — the caregiver is already in the home.
- The switch is not automatic; a request must be submitted.
The foreign-caregiver track
The request is filed as a direct employer of a caregiver (foreign worker) requesting most of the benefit in cash. Note the official wording — most of the benefit, not necessarily all of it. Per the National Insurance Institute, no documents need to be attached — the employment information updates directly from the Population and Immigration Authority.
What determines the amount
- The benefit level set by National Insurance, based on the ADL dependency assessment.
- Higher dependency generally means a higher level and a higher amount.
- Amounts are updated periodically — always check the current figure with National Insurance.
What it covers — and what it does not
- It helps fund the wage but does not necessarily cover the full cost of employment.
- Pocket money, medical insurance, deposit contributions and authority fees are additional.
- Bureau placement and monthly care fees are not part of the benefit.
